A copy will be forwarded to your email address; once received, please print it off and enclose it in the package(s) with your screens before sending them to us.

Submitting this form and sending your screens to us indicates that you have read and accepted both our general requirements and our terms and conditions for refurbishment.
We are only able to accept original screens for refurbishment (including previously-refurbished/recycled ones). We cannot accept non-original ("copy") screens from third-party manufacturers- please read our article explaining the difference if necessary, or contact us if you still require further details.
"Screen" is screen-only refurbishment, "Device" is for the whole device. Please make sure you understand the difference between our "Screen" and "Device" refurbishment options before posting.
